I/we, the undersigned hereby certify that I/we are the parent or legal guardian of the player and hereby give my/our permission for the staff of the camp to seek appropriate medical attention to be given in the event of an accident or injury or illness.  I/we will be responsible for any and all costs of the medical attention and treatment and will not hold Jay Miller, AFUSA or the Gwinnett Department of Parks and Recreation liable for  any injury occurring at the camp.
